What care navigators do, in plain language
When a parent or partner starts needing more help, the care landscape can feel overwhelming. The right choice depends on medical needs, cognition, finances, family support, and — often most importantly — the older adult's preferences.
This guide walks through the main paths in plain language. Most families end up combining several over time: home care now, assisted living later, and skilled nursing only if and when medically required.
There's no single correct path. Reassess every 6–12 months, or whenever there's a hospital stay, new diagnosis, or significant change in function.
